Print out this two-page form. Complete the form and send it to your VISTA 2 Finance Ambassador - Sue J. by 6/30/07.
Please also hold on to the forms you receive from each troop until I confirm what happens to those forms. I think you will ultimately send them to me to send on to our liaison at the Hub, but I am not sure on this yet. Please try to follow up to be sure you get a form from each troop in your Community.
